Assistive Technology Process

Assistive Technology follows a process that can be new for people that have not gone through it before.  Below is a presentation that will ask you a series of questions that will take you through the AT Process.

Assistive Technology Forms

When you go through the AT process, there are a number of different forms that are referenced throughout.  One of the first forms - the one that starts the process, is the Assistive Technology Request Form.  The AT Request form is available on the Main AT page - you can also access it by clicking here

You'll need the following information to complete this form:

    1. Student Name

    2. Student ID Number

    3. Case Manager Name (if not you)

Before an Assistive Technology Collaboration Meeting, you may receive a link to complete a Google Form: "Assistive Technology Information Gathering Tool.

This form is completed by the student's Academic Teachers and Case Manager as a way to get a better picture of the student as a learner.  It helps us avoid needing to pull all of the teachers from their classes in order to bring the entire team together for a meeting.
