• Life Insurance (Voluntary)
     
    District 308 offers employees the option to purchase voluntary life insurance benefits at their cost. Please refer to the voluntary group life insurance summary plan description for more information.  (New Summary Plan Information Coming Soon!)

    The district offers coverage through Dearborn National. Employees can elect coverage up to five times their annual base salary up to $500,000 subject to medical underwriting approval. Coverage is also available for spouse and children.

    To update your beneficiary information on your voluntary life insurance policy, please contact Glenn Campos in Benefits for instructions.  
     
     
     
    If you wish to continue voluntary Life and AD&D coverage after your benefits eligibility has ended, please complete a Portability Life and AD&D Application and return it to Human Resources within 30 days of your benefits end date.
